This form is designed to provide the Eckerd College community, which includes, but is not limited to students, student organizations, faculty, administrators, staff, guests, visitors, invitees, and campers with an on-line method to report specific information related to an alleged incident(s) of policy violations.
Please note that the College can address submissions most completely when we are able to communicate with people reporting. The benefits of direct reporting include a greater ability for Eckerd to understand the situation, provide resources, support, and options to those reporting, and to address the behavior fully and accurately.
We do accept anonymous reports. Those wishing to report anonymously may submit this form without identifying information. If you choose to submit a report anonymously, please note that Eckerd will strive to fully respond, but anonymous reporting significantly limits our ability to do so.
Furthermore, please be aware that the individuals involved may be afforded certain privacy rights under the Family Educational Rights and Privacy Act (FERPA). The Office of Community Standards strives to maintain information provided as private, to the extent in which the law allows. Please note that students have the right to inspect any information submitted regarding this matter. Our office may disclose information to appropriate college staff for evaluation and/or response purposes.
